Please print the appropriate:
Medical Action Plans: (and take to your doctor’s office)
- Asthma Action Plan
- Seizure Action Plan
- Food Allergy Action Plan
- Bee Sting Allergy Action Plan
WUSD Medication Authorization: (take to your doctor’s office, parent must sign too)
- Medication Administration Authorization Form, one for each medication to be taken at school
These forms need to be completed and returned to your child's school office before school starts.

2025-26 Illness Policy
Dear WUSD Families:
We are off to a great start of the year, but wanted to check in about our guidelines regarding what to do when your student is sick. We follow the California Department of Health (CDPH) Guidelines, which are outlined below. However, no matter what the specifics of the guidance are, it should be pointed out that the CDPH states that a child should always stay home (or go home) from child care or school when any new illness or symptom:
- prevents them from participating meaningfully in child care or school activities; OR
- results in a need for care that is greater than the staff can provide without compromising the health and safety of other children.
In addition, under California law, a child may be required to stay home (or go home) from school in specified circumstances where an apparent illness presents a significant risk to other children or school personnel. (Education Code § 49213; Education Code § 49451.)
In other words, even if you think your students symptoms fall under guidance that would allow them to be at school, if your child is struggling to participate, clearly feels lousy, and/or is requiring care from school staff, you will be asked to take (or keep) them home until they are able to participate in school activities. Use the sign/symptom guidance table below ONLY if your child can participate meaningfully in child care or school activities; AND school staff can provide appropriate care without compromising the health and safety of other children.
If these criteria are not met, the child should be sent (or kept at) home.
Other considerations:
- Face masks should be considered for children 2 years of age and older with respiratory symptoms (e.g., runny nose, cough, sore throat) while awake to lower the chances of getting others sick. ​See icon where relevant. Please note, face masks should never be used on babies or children under 2 years of age.
- For certain symptoms (e.g., diarrhea, vomiting), special attention to hand washing is recommended. See icon where relevant.
- If a child has been diagnosed with a specific illness or develops compatible symptoms after an exposure to a communicable disease or during an outbreak of a communicable disease, follow instructions for when the child may return to child care or school based on that illness.
- Additionally, certain symptoms warrant medical attention, sometimes immediately. See the icon. Note that this list is NOT exhaustive, and there may be other situations where the child care or school should seek medical attention for the child.
